If you would like an assessment for exam access arrangements only, typically used for 11+ exams, GCSEs and A Levels, please message Hannah on 07540529661. Please do not fill in these referral forms.

Refer your child

 

This is the quickest way to make a referral.

Click the button below to complete and submit a Referral Form.  These forms help me to plan what tests to use.

An assessment and report is £1795. Optional services are listed on the Referral Form.

There is also a Teacher Form to be completed by your child’s education setting, and this will be automatically emailed to them after you submit your referral. If your child is home educated/receives EOTAS, please put the contact details of the person/organisation who is legally responsible for their education.

I do not carry out assessments without involvement from a child’s education setting. If you have any concerns with regards to this, please message or telephone to discuss further.

Ensure you have other documents ready to upload, such as reports from school or other professionals, as you cannot save the referral form (you will be emailed a copy after submitting the referral).

What's included in the assessment and report?

  • Your child’s strengths and needs in the following areas*:
    • Intelligence
    • Processing and memory
    • Literacy and maths
    • Attention and other executive function skills
    • Social skills, language and communication
    • Motor skills and sensory processing
    • Emotions and behaviours
    • Everyday life skills
    • Expressive arts
    • *It is not always possible to cover all areas of development, eg if a child is very young or becomes dysregulated, but I will always try to explore as much as possible.
  • Recommendations are made for supporting your child at school, eg teaching strategies, useful resources, exam access arrangements and additional targeted teaching.
  • Where relevant: Advice for school transitions, supporting mental health and medical needs (eg Tourettes, epilepsy and diabetes) or how to help if your child has been bereaved or experienced bullying.
  • If there is evidence of dyslexia, dyscalculia, general learning difficulties or high learning potential, these will be diagnosed.
  • If there are indicators of autism, ADHD, dyspraxia (DCD) or any other differences, advice will be given for support at school and which other professionals to refer your child to for diagnostic assessments.
  • Where relevant: Advice and recommendations for an EHCP, eg individual support, environmental adjustments and staff training. Information is specific and quantifiable.

Where will the assessment take place?

Most assessments take place at my home (J22 of the M25), but some children require their assessments to take place at their home or school. I will confirm the location after reading the referral forms.

Is a school observation required?

If there are concerns regarding social skills or aggressive/unusual behaviours, then an observation at school is advised, otherwise this is optional. It may not be possible for me to observe your child if their school location is further than I travel for work. I will advise further once I have read the referral forms.

When will I be offered an assessment date?

Assessment dates will be offered after the main referral form has been completed and reviewed by the EdPsych4Kids team (usually 2-3 days), and all relevant additional documents have been received, such as reports from other professionals an the strengths and difficulties questionnaire (emailed to you after you submit your child’s referral).

When will I receive my child's report?

Reports usually take 4 weeks (excluding school holidays). You will be contacted if there are any unexpected delays.

Do you offer tribunal services?

No I do not offer tribunal services.
